Lafayette Man Killed and Three Injured in Head on Crash

Contributing factors are still under investigation.

(LAFAYETTE, Ore.) - One man was killed and three others injured about 2:40 am, early Sunday morning, August 28, 2016. The two vehicle crash happened on Highway 99W at milepost 31.5, near Lafayette.

Preliminary investigation revealed a 1997 Toyota Camry was traveling southbound Highway 99W when it crossed the centerline and collided head-on with a northbound 2008 Chevrolet Trailblazer.

The driver of the Toyota, 21-year old Jeronimo Huapoe-Morales, of Lafayette, was pronounced deceased on the scene. Two passengers with him in the Toyota were also taken to the hospital for their injuries.

The driver of the Chevrolet, 31-year old Cody M Kliese-Hartzong, of Dundee, was taken by ground ambulance to Oregon Health and Sciences University.

All three occupants in the Toyota were not using their safety belts.

The contributing factors into the crash are still under investigation. OSP Troopers were assisted by the Yamhill County Sheriff's Office, McMinnville Police, Lafayette Fire, Dayton Fire, McMinnville Fire, Tualatin Valley Fire and Rescue and the Oregon Department of Transportation.
